HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL AND HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Commitment

Compass of Integrity Healthcare Consultants, LLC ("we") is committed to protecting the privacy of your Protected Health Information ("PHI") in accordance with the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), as amended.

How We May Use and Disclose Your PHI

  • For Treatment. To coordinate non-medical care and, with your authorization, to share information with your medical providers.
  • For Payment. To bill and obtain payment from you, your representative, or your long-term care insurer.
  • For Health Care Operations. Quality assurance, caregiver training, scheduling, and internal administration.
  • As Required by Law. When disclosure is required by federal, state, or local law.
  • To Avert a Serious Threat. To prevent imminent harm to your or another's health or safety.
  • To Family and Friends. Where you have not objected, to those involved in your care.

Uses and Disclosures That Require Your Authorization

Most uses and disclosures of psychotherapy notes, marketing communications, and the sale of PHI require your written authorization. You may revoke an authorization at any time in writing, except to the extent we have already relied on it.

Your Rights

  • Right to Inspect and Copy. You may request access to PHI we maintain about you.
  • Right to Amend. You may request that we amend PHI you believe to be inaccurate or incomplete.
  • Right to an Accounting of Disclosures. You may request a list of certain disclosures we made of your PHI.
  • Right to Request Restrictions. You may request limits on how we use or disclose PHI.
  • Right to Confidential Communications. You may request that we contact you in a specific way or location.
  • Right to a Paper Copy. You may obtain a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
  • Right to be Notified of a Breach. You will be notified following any breach of your unsecured PHI.

Our Responsibilities

  • We are required by law to maintain the privacy and security of your PHI.
  • We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this Notice and give you a copy of it.

Changes to This Notice

We reserve the right to change this Notice and to make the revised notice effective for all PHI we maintain. The current Notice will always be available on this website and at our office.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights at https://www.hhs.gov/ocr/. We will not retaliate against you for filing a complaint.
